By submitting your repair request, you are confirming that you reside at the property named above and have requested the above described repair.  You understand and acknowledge that if the repair professional determines the repair is needed due to your negligence, you will be responsible for the invoice in full.  The invoice will be due immediately upon receipt.  If you are not available at the time of repair, you give permission for the premises to be entered for the repair to be made.

Your repair request will be submitted immediately to our system and your property manager will be contacted reguarding your request.  This does not guarantee the property owner will approve the repair.  Your property manager will discuss this with you.
